According to the arrest warrant obtained by WFAA, Hernandez began "acting strangely" at the hospital and accused his girlfriend of cheating on him. Later, the suspect went on to search the room to check if anyone else was in the room. Hernandez then pulled out a handgun and hit his girlfriend multiple times in the head with it, the affidavit said. “We are both going to die today” and “whoever comes in this room is going to die with us," yelled the suspect.

After making several "ominous phone calls and texts to his family," Hernandez allegedly continued his "strange behavior," reports CBS News.

This is when Jacqueline Pokuaa, the first victim, entered the room and was immediately shot dead by Hernandez. Pokuaa, according to Ghannaweb.com, is a woman of Ghanna origin. Later, the second victim, Annette Flowers, then looked into the room to see the first victim's body. This was when Hernandez fatally shot her. This is when Sgt Rangel shot Hernandez in the right leg. The 30-year-old was then taken to another hospital for treatment. According to Fox4news, Hernandez has a lengthy rap sheet that includes charges of burglary, assault of a public servant, robbery, and theft. 

During a press conference on Monday, October 24, 2022, Dallas Police Chief Eddie Garcia said, "We lost two healthcare workers who were going about their day helping others." He then appreciated Sgt Rangel and said, "There is no doubt he saved lives by preventing Hernandez from leaving the room."

"A violent individual like this should not have been on ankle monitoring. He should have still been in-custody," said Garcia, adding, "Ankle monitors on violent criminals are useless.

Hernandez was previously in jail for pleading guilty to an armed robbery in 2015 and receiving an eight-year sentence, reported the New York Post. Meanwhile, Dallas Police Chief Eddie Garcia tweeted, "We @DallasPD are grateful for the support and care provided to our officers by @mhshospitals. Our thoughts are with staff and victims of today’s events.We will do EVERYTHING to assist in this investigation.This is a tragedy, and an abhorrent failure of our criminal justice system."

The Methodist Dallas Medical Center also noted in an official statement, "The Methodist Health System Family is heartbroken at the loss of two of our beloved team members. Our entire organization is grieving this unimaginable tragedy. During this devastating time, we want to ensure our patients and employees that Methodist Dallas Medical Center is safe, and there is no ongoing threat."
